Chelsea Cooke
Lead Support Coordinator
Chelsea is Collective Pathways’ Lead Support Coordinator and has spent the past ten years working in the NDIS sector. She is an integral part of the team, known for her extensive knowledge, unwavering advocacy, and the genuine care she brings to every participant and family she supports.
Chelsea leads with her heart, believing everyone deserves someone in their corner who will listen, advocate, and never stop striving for the best possible outcome. She takes the time to truly understand each person’s goals, challenges, and what matters most to them, building genuine relationships founded on trust, honesty, and compassion.
With both professional and lived experience, Chelsea understands that every journey is different and that navigating the disability sector can often feel overwhelming. Whether she’s coordinating complex supports, celebrating milestones, or guiding families through challenging times, she is committed to removing barriers, creating opportunities, and empowering participants to make informed decisions with confidence.
As Lead Support Coordinator, Chelsea is equally passionate about supporting her team. She fosters a culture of person-centred practice, believing the best outcomes are achieved when people feel genuinely heard, valued, and supported.
Outside of work, Chelsea loves spending time with her fiancé, children, and their much-loved pets. Whether she’s exploring nature, enjoying family board game nights, cheering from the sidelines of the basketball court, or sharing a coffee and a chat, Chelsea brings the same warmth, positivity, and genuine care into her personal life as she does her work.
